When you vi­sit my web­site, my web ser­ver (the com­pu­ter on which this web­site is stored) au­to­ma­ti­cal­ly saves data such as the IP ad­dress of your de­vice, the ad­dres­ses of the sub­pages vi­si­ted, de­tails of your brow­ser, and the date and time. Ac­cord­ing to the Psy­cho­the­ra­py Act §16, psy­cho­the­ra­pists are re­qui­red to keep re­cords. For this pur­po­se, I collect and pro­cess the fol­lo­wing data from you: Per­so­nal data (tit­le; sa­lutati­on; last name and first name; ad­dress, city, pos­tal code; te­le­pho­ne num­ber). Spe­cial ca­te­go­ry per­so­nal data (health data): Psy­cho­the­ra­pists must re­cord every psy­cho­the­ra­peu­tic mea­su­re ta­ken, in­clu­ding: Histo­ry of the pro­blem and any ill­ness, as well as pre­vious dia­gno­ses and the cour­se of the ill­ness; start, cour­se, and end of the psy­cho­the­ra­peu­tic ser­vices; type and extent of dia­gnostic ser­vices, coun­se­ling or tre­at­ment in­ter­ven­ti­ons, and re­sults of any eva­lua­ti­on; agreed fee and any other agree­ments from the tre­at­ment con­tract, es­pe­ci­al­ly with any le­gal re­p­re­sen­ta­ti­ves; pro­vi­ded cla­ri­fi­ca­ti­on steps and do­cu­men­ted in­for­ma­ti­on; con­sul­ta­ti­ons with pro­fes­sio­nals or other health pro­fes­si­ons; trans­mis­si­on of data and in­for­ma­ti­on to third par­ties, es­pe­ci­al­ly to health insuran­ce car­ri­ers; any re­com­men­da­ti­ons for sup­ple­men­ta­ry me­di­cal, cli­ni­cal-psy­cho­lo­gi­cal, health-psy­cho­lo­gi­cal, or mu­sic the­ra­py ser­vices or other cla­ri­fi­ca­ti­ons; ac­cess to do­cu­men­ta­ti­on and rea­sons for re­fu­sing ac­cess to do­cu­men­ta­ti­on.

Sto­rage Du­ra­ti­on

The le­gal re­ten­ti­on pe­ri­od for psy­cho­the­ra­pists re­gar­ding per­so­nal data re­qui­res that this data must be ar­chi­ved for at least 10 ye­ars af­ter the end of psy­cho­the­ra­py.
